Brent, Central London, Ealing, Hammersmith and Fulham, Harrow, Hillingdon, Hounslow, and West London Clinical Commissioning Groups is transferring to the new CCG – North West London Clinical Commissioning Group on 1 April 2021. The new Clinical Commissioning Group will become the new data controller.

Emerging Communities Engagement

Nepalese community healthcare programme

This group aims to tackle the issues surrounding access to services indicating a lack of understanding of the health care system and services in Hounslow that are available to sufferers of illness and long-term health conditions. Ultimately, it will provide an understanding of what they can do to effectively ‘self-manage’ their condition and reduce the risk of developing complications, for example encouraging them to lead an active and healthy lifestyle or making changes to their diet.

The project will discuss ways in which the Nepalese community can be connected to the various services which are available to patients in Hounslow that can help them self-manage their illnesses better i.e. NHS 111, Diabetes Service, Expert Patient Programme, Conversation maps, Vascular Health Care, Hounslow Stop Smoking Service etc. It will also be able to acquaint the community to activities which have been organised and provided by Public Health that they can access, such as group walks, yoga activities and healthy eating/cooking clubs etc.

It will aim to get a better understanding of the common health ailments within the Nepalese community in the borough of Hounslow by running focus groups and tailor-made awareness sessions on how they can ‘self-manage’ their illnesses and health conditions with participants from Hounslow borough.
